Registration for the 2008-2009 Soccer Season
Holland G. Mangum Recreation Complex,

REGISTRATION DATES:
August 9th 9:00 am - 1:00 pm
August 13th 6:00 pm - 8:00 pm
August 25th 6:00 pm - 8:00 pm
AGES 4 TO 18
(Must be born on or before July 31, 2004)
The following are required at registration:

Seminole recreation card VALID thru 2/28/09*

Copy of a State Certified Birth Certificate

Wallet size photo
-
A check payable to SSSSA (be prepared to bring another check if purchasing a rec card)

Player Information form
-
Medical Release form
-
Please bring your child (uniforms will be sized at registration)
Children cannot be registered without these documents
REGISTRATION FEE:
(Includes Team shirt, shorts, socks, trophy, roller skating party and an end of season picnic)
Under 6: $95.00
Under 8 - Under 19: $105.00
$10.00 Discount for Additional Children
Recreation Cards are free to Seminole Residents. Non-Residents are $120.00.
Stop by the front office at the
Recreation
Center to obtain one. With the Rec. Card you can enjoy all the benefits the
Rec.
Center has to offer. Card is valid for one year from date of purchase.
Practice will start the first week of October. Games begin the first Saturday of November.
